Um Percurso de aprendizagem em cuidados paliativos

Abstract

O presente relatório de estágio visa apresentar todo um percurso do estágio de natureza profissional desenvolvido no serviço de cuidados paliativos do Instituto Português de Oncologia do Porto, Francisco Gentil, com o objetivo de desenvolver competências na área dos cuidados paliativos. Os cuidados paliativos, são cuidados prestados a doentes com doença incurável e/ou grave, e suas famílias, realizados por uma equipa multidisciplinar, quando a doença já não responde a tratamentos curativos. A construção deste relatório assenta em três domínios de intervenção - a prestação de cuidados, a formação e a gestão de cuidados. Nele procuramos descrever, analisar e refletir sobre as atividades e competências desenvolvidas ao longo do estágio, em contexto de internamento, assistência domiciliária e consulta externa de uma unidade de cuidados paliativos de nível III. A nível da prestação de cuidados, área que privilegiamos durante este percurso, procuramos descrever a intervenção perante o doente em intenso sofrimento, em fase de fim de vida, através da identificação dos problemas, planeamento de intervenções que visem uma resposta efetiva às necessidades identificadas, executar e avaliar os seus resultados, numa perspetiva multidisciplinar. As competências desenvolvidas são essencialmente na área da comunicação, do trabalho em equipa, do apoio aos doentes/famílias em intenso sofrimento e/ou em processo de luto e do controlo de sintomas. A metodologia utilizada na construção deste relatório foi sobretudo uma metodologia descritiva e analítica. A realização deste estágio possibilitou-nos o desenvolvimento de uma maior autonomia e capacidade na tomada de decisão no âmbito da prestação de cuidados à pessoa/família em contexto de cuidados paliativos.

This report serves the purpose of presenting the course of my professional internship which took place in the palliative care department of the Portuguese Oncology Institute in Oporto, Francisco Gentil, the objective of which was to develop competences in the area of palliative care. Palliative care is care given by a multi-disciplined team not only to patients with an incurable disease and/or a serious one, but also to their families, when the patient’s illness no longer responds to curable treatments. The structure of this report focuses on three areas of intervention – provision of services, training and management of care. Here we try to describe, analyze and reflect upon the activities and competences developed during the internship, in the context of hospitalization, home care and external consultation of a palliative care level three unit.With regard to the provision of services – the area we gave privilege to during this experience - we sought to describe the intervention to the suffering patient in the final stages of his/her life, thereby identifying the problems, planning the intervention to get the most effective response to the necessities which were identified, execute it and evaluate the results in a multi-disciplined perspective. The competences developed are essentially in the communication area, the working in a team, the support to the patients and their families who are suffering intensely and/or in the process of mourning and finally the control of the symptoms. The methodology used in the elaboration of this report was mainly a descriptive and analytical one.The fulfillment of this internship allowed us to develop more autonomy and capacity in making decisions in the area of provision of services to a person or their family in the context of palliative care.
