A Perspetiva dos profissionais de saúde no cuidar do doente em fim de vida

Abstract

A perspetiva do cuidar da pessoa em fim de vida modificou-se com o tempo. O cuidar esteve muito tempo associado a cuidados no seio familiar, com proteção, apoio, afetividade e responsabilidade. Hoje, a pessoa sente-se omnipotente, admitindo a fuga à morte. O avanço da medicina e das medidas terapêuticas conduzem a pessoa para o hospital, envolvida por alta tecnologia, mas distanciada do profissional de saúde. O cuidar é fragmentado, despoletando sofrimento. Visando perceber a perspetiva dos profissionais de saúde relativamente ao cuidar do doente em fim de vida, colocou-se a questão de investigação: “Qual a perspetiva dos profissionais de saúde relativamente ao cuidar do doente em fim de vida?”, com a finalidade de contribuir para cuidados inseridos na filosofia dos cuidados paliativos. Objetivo Geral: Perceber a perspetiva dos profissionais de saúde relativamente ao cuidar do doente em fim de vida. Metodologia: abordagem qualitativa, estudo exploratório; população: profissionais de saúde de um hospital do Alto Minho; método de colheita de dados: entrevistas semiestruturadas, submetidas a análise de conteúdo, emergindo áreas temáticas, categorias e subcategorias. Principais Achados: Os profissionais de saúde apresentam distintas representações do fim de vida. Experienciam uma diversidade de emoções e sentimentos perante o doente em fim de vida. O apoio, controlo de sintomas, cuidados de conforto, avaliação e monitorização dos cuidados, preparação do regresso a casa, são fundamentais para responder às necessidades do doente e família. Intervenções biomédicas, avaliar necessidades físicas, preparar o regresso a casa no dia da alta, défice de formação, falta de disponibilidade e falta de tomada de decisão em equipa de saúde, são apontados como obstáculos aos cuidados de qualidade. Como necessidades destacam: existência de equipas/ unidades de cuidados paliativos, maior trabalho em equipa, apoio psicológico, formação em cuidados paliativos, existência de recursos adequados, desenvolvimento de habilidades comunicacionais, uniformização dos cuidados e maior disponibilidade. A metodologia partilhada é utilizada na procura da transformação do cuidar. Informar o doente e família, integra-los e apoia-los na tomada de decisão, são estratégias dos profissionais durante a educação para saúde. São vários os dilemas sentidos no cuidar do doente em fim de vida e família. Conclusão: Cuidar da pessoa em fim de vida, que vive até morrer e não morre com o diagnóstico incurável, constitui-se como um alerta para todas as organizações de saúde. Cuidar do doente em fim de vida e família exige que os profissionais de saúde foquem o cuidar na pessoa.

The perspective on caring for patients at the end of life has changed over time. For a long time, this care was associated with the patient being looked after by the family at home, with the benefit of the family’s protection, support, dedication and sense of responsibility towards the patient. Today, people feel all-powerful, wanting to run away from death. Advances in medicine and therapeutic treatments mean that the patient is treated in hospital, surrounded by the latest in medical technology, but alienated from the healthcare professional. Patient care is fragmented, leading to suffering. Seeking insights into the perspective of healthcare professionals in relation to the care of patients at the end of life led to the research question: “What is the perspective of healthcare professionals in relation to the care of the patient at the end of life?”, with the aim of contributing to the care of patients within the philosophy of palliative care. General aim: To gain an understanding of the perspective of healthcare professionals towards end of life care of patients. Methodology: qualitative approach, exploratory study; population: healthcare professionals at a hospital in “Alto Minho”; method of data collection: semi-structured interviews, whose content has been analyzed to reveal certain themes, categories and subcategories. Principal findings: Healthcare professionals have different approaches towards the idea of end of life. They feel a variety of different emotions and feelings towards patients at the end of life. Providing support, managing symptoms, looking after the well-being of patients, evaluating and monitoring the care given and preparing patients for the return to their homes are a fundamental part of responding to the needs of patients and their families. Biomedical interventions, the assessing of physical needs, preparing the patient for returning home on the day of discharge from hospital, lack of adequate training, lack of time and failure on the part of the healthcare team to take decisions are identified as obstacles to quality care provision. The need for the following is identified: palliative-care teams/ units, improved team work, psychological support, training in palliative care, adequate resources, improvements in effective communication, the standardization the level of care, and more availability. This methodology is used in the search for the transformation of patient care. Keeping patients and their families informed, and involving them and supporting them in the decision-making process, are strategies used by healthcare professionals to educate the patients and families about health matters. There are a variety of dilemmas facing health-care professionals in caring for patients at the end of life and their families. Conclusion: Caring for someone at the end of their life, who lives life to the full until they die and whose life doesn’t end once they are diagnosed with a terminal illness, should be a wake-up call to all healthcare organizations. Looking after the patient at the end of life and their family demands that health professionals focus on the care of the person.
