Intervenção do enfermeiro especialista em enfermagem de reabilitação numa ULDM: perceções dos enfermeiros

Abstract

A Unidade de Longa Duração e Manutenção, da Rede Nacional de Cuidados Continuados Integrados, visa prestar cuidados a pessoas com doenças/processos crónicos, com diferentes níveis de dependência, impedindo o agravamento da situação de dependência, promovendo a melhoria/manutenção das capacidades funcionais da pessoa. A Ordem dos Enfermeiros considera que, para tal, seria fundamental que a ULDM fosse dotada de enfermeiro especialista em enfermagem de reabilitação, porque se dirige à recuperação/manutenção ou adaptação funcional da pessoa, à capacitação do prestador de cuidados, com o objetivo de preparar o regresso ao domicílio, resultando ganhos em saúde significativos. Neste contexto surge o presente estudo de investigação, cuja finalidade é contribuir para a compreensão do processo de cuidados no âmbito da intervenção do enfermeiro especialista em enfermagem de reabilitação numa ULDM. Este estudo, intitulado como “Intervenção do Enfermeiro Especialista em Enfermagem de Reabilitação numa ULDM: Perceções dos Enfermeiros”, foi realizado em duas ULDM da região do norte e teve como objetivo geral: conhecer as perceções dos enfermeiros, relativamente à intervenção do enfermeiro especialista em enfermagem de reabilitação numa ULDM, e como objetivos específicos: conhecer a opinião dos enfermeiros de cuidados gerais relativamente à intervenção do enfermeiro especialista em enfermagem de reabilitação numa ULDM; conhecer a opinião dos enfermeiros especialistas em enfermagem de reabilitação relativamente à sua intervenção numa ULDM; identificar contributos dos cuidados de enfermagem de reabilitação numa ULDM.Trata-se de um estudo qualitativo de natureza exploratório-descritiva, foram utilizadas como fontes de colheita de dados a entrevista semiestruturada e a observação não participada, em meio natural. A análise dos dados foi feita com base na técnica de análise de conteúdo segundo Sampieri et al. (2006), com recurso à triangulação de dados, das diferentes fontes de colheitas de dados. Os resultados obtidos permitem-nos concluir que a intervenção do enfermeiro de reabilitação numa ULDM é muito importante na equipa multidisciplinar e na prestação de cuidados especializados. Os enfermeiros salientam que o foco da intervenção do enfermeiro de reabilitação na equipa multidisciplinar é a equipa de enfermagem, a pessoa e o prestador de cuidados. Salientando também, que o enfermeiro de reabilitação presta cuidados especializados, sendo o foco da sua intervenção centrada na avaliação da funcionalidade e no diagnóstico de alterações, na conceção, implementação/execução de planos de intervenção e na promoção da inclusão da pessoa na comunidade. Constatamos ainda que a intervenção do enfermeiro de reabilitação contribui para a obtenção de ganhos em saúde, relacionados com: a satisfação da pessoa, promoção do bem-estar e autocuidado, prevenção de complicações, promoção da saúde, reeducação e readaptação funcional da pessoa. Os resultados destacam ainda, que a intervenção do enfermeiro de reabilitação é percebida pelos pares como fundamental numa ULDM.

The Unit of Long-Term and Maintenance, of the National Network of Continued Integrated Care, aims to provide care for people with chronic illnesses, with different levels of dependence, preventing the aggravation of the situation of dependence, and promoting the improvement/maintenance of the functional abilities of the person (Decreto-Lei nº 101/2006, 2006). The Order of Nurses (2010) considers that it would be essential that the unit of long-term and maintenance was endowed with a nurse specialist in rehabilitation nursing because it directs the restoration/maintenance and functional adaptation of the person, the training of the caregiver, with the goal of preparing them to return home, resulting in significant health gains. In this context arises the present research study, whose purpose is to contribute to the understanding of the process of care, in the ambit of the intervention of the nurse specialist in rehabilitation nursing in the unit of long-term and maintenance. This study, titled "Intervention of the Nurse Specialist in Rehabilitation Nursing in a Unit of Long-Term and Maintenance: Perceptions of Nurses" was conducted in two units of long-term and maintenance of the northern region and had the general objective: to get to know the perceptions of nurses, in relation to the intervention of the nurse specialist in rehabilitation nursing in a unit of long-term and maintenance, and as specific objectives: to get to know the opinion of nurses of general care about the intervention of the nurse specialist in rehabilitation nursing in a unit of long-term and maintenance; to get to know the opinion of the nurse specialist in rehabilitation nursing about their intervention in a unit of long-term and maintenance; and identify contributions of rehabilitation nursing in a unit of long-term and maintenance.It is a qualitative study of exploratory-descriptive nature, were used as sources of data collection the semi-structured interview and the observation not participated, in natural environment. The analysis of the data collected was based on the technique of content analysis according to Sampieri, Collado and Lucio (2006), using the triangulation of data, from the different sources of data collected. The results allow us to conclude that the intervention of the rehabilitation nurse in a unit of long-term and maintenance is very important in the multidisciplinary team and in the provision of specialized care. The nurses highlight that the focus of the intervention of the rehabilitation nurse in a multidisciplinary team is: the nursing staff, the person and the caregiver. Highlighting also, that the rehabilitation nurse provides specialist care, being the focus of his intervention focused on evaluating the functionality and diagnosis of changes, in the design, implementation / execution of plans, and in promoting the inclusion of the person in the community. We also acknowledge that the intervention of the rehabilitation nurse contributes to achieving health gains, related to: customer satisfaction, promotion of wellness and self-care, prevention of complications, health promotion, and re-education and functional rehabilitation of the person; The results also highlight that the intervention of a rehabilitation nurse is perceived by peers as a fundamental in a unit of long-term and maintenance.
