Encargos com o acidente vascular cerebral no Alto Minho

Os custos das doenças cardiovasculares são maiores em Portugal, que noutros países europeus, porque a incidência do AVC continua a ser a mais alta. A sub-região do Minho Lima, não diverge muito desta realidade. O AVC apresenta um elevado consumo de recursos e de encargos no SNS e portanto a sociedade. O estudo desenvolvido aplicou a metodologia proposta na análise económica - Custo da Doença - CdD, na literatura conhecido pelo “Cost of Ilness” com o objectivo de estimar os encargos com o AVC na área de influência da Unidade Local de Saúde do Alto Minho, E.P.E,. Teve como propósito fundamental medir e avaliar o impacto da doença numa perspectiva societal, ao nível dos recursos económicos que lhe são afectados. O valor global dos encargos por AVC, em 2009 na sub-região do Alto Minho foi estimado em cerca de 8 859 815.50 Milhões de Euros à sociedade e economia portuguesa. - Os custos directos estimados com o AVC incluem os relacionados com a utilização dos serviços de saúde nos serviços de Medicina Interna da ULSAM, e os custos suportados pelo sistema de saúde no período após a alta. Dos quais 7 114 796.89 ! Milhões de Euros em custos totais directos, representando 80,3% dos custos totais. -Os custos indirectos serão considerados os custos relacionados com a perda de produtividade dos trabalhadores vivos com incapacidade em idade produtiva/ activa (com menos de 65 anos), resultante do abandono definitivo do trabalho. Ainda foram calculados os custos ocorridos por morte prematura, confrontados com a perda de produtividade dos trabalhadores com menos de 65 anos, resultante do abandono definitivo do trabalho, por mortalidade hospitalar. 1 745 018,61 Milhões de Euros em custos totais indirectos, representando 19,7%. O presente estudo demonstrou que a ocorrência do AVC arrasta consigo perdas económicas elevadas para o país. Os resultados indicam que a implementação de estratégias para prevenir ou reduzir a incidência e prevalência do AVC em Portugal poderia gerar importantes poupanças na utilização de recursos no sistema de saúde para além dos ganhos associado à superior produtividade. Ainda que reconhecendo que a estimativa efectuada não releva a totalidade dos custos em consequência do AVC, dada a dificuldade em atribuir um valor monetário à dor, ao desconforto e a toda a carga que esta doença gera na qualidade de vida dos doentes e das suas famílias, parecenos que este exercício pode permitir valorizar o impacto económico resultante da redução desta patologia na população do Alto-Minho.

The review and the results of the economic evaluation represent one needful tool of support at the level of decisions. The cost of illness is an important element of the health economics, because meets the global consequences of a disease in one society. The cost of the cardiovascular diseases in Portugal is bigger than in other European countries because the incidence of CVA’s continues to be the highest. The sub-region of Minho Lima is not far away of this reality. CVA represents a high resources consume and charges for the NHS and therefore, for the society. The study developed applied the methodology proposed by economical analysis– Custo da Doença - CdD, known in literature as “Cost of Illness” with the objective of estimate the charges with Cerebral Vascular Accident’s in the area served by Unidade Local de Saúde do Alto Minho, E.P.E,. It had as major purpose to measure and evaluate the impact of the disease on a social perspective, at the level of the economic resources that are affected. The global value of the charges with stroke, during 2009 in the sub-region of High Minho was estimated about 8 859 815.50 Millions of Euros from the Portuguese economy and society. -The direct costs estimated with CVA’s include the ones related with the using of the health services at medical admission wards of ULSAM and the supported costs by the heath system at the post discharge period. 7 114 796.89 Millions of Euros are the total direct costs, representing 80,3% of the total costs. -The indirect costs: are considered the costs related with the loss of productivity of the live workers with disability on productive/active age (less than 65 years old), resultant of the definitive absence from work. There were also calculated the costs caused by the premature death, confronted with the loss of productivity of the workers with less than 65 years old, resultant of the abandonment definitive from work, by hospital death. 1 745 018,61 Millions of Euros on total indirect costs, representing 19,7 %. The present study demonstrated that the occurrence of CVA’s drags along high economic losses to this country. The results point that the implementation of strategies to prevent or reduce the incidence and prevalence of CVA’s in Portugal could generate important savings in the utilization of resources at the health system, therefore the earnings related with the high productivity. Even recognizing that this estimation does not reveal the totality of the costs in consequence of one CVA because of the difficulty to attribute a monetary value to pain, discomfort and all the charge that this disease creates in the quality of life of patients and their families, it looks like this exercise could improve the economic impact caused by the reduction of this pathology in the population of High-Minho.
