Title: Funcionalidade, aptidão física e qualidade de vida do idoso da região Minho-Lima: contributos para a enfermagem de reabilitação

Abstract: Face ao envelhecimento, as atuais necessidades de saúde da população idosa representam uma oportunidade para o enfermeiro especialista em Enfermagem de Reabilitação fazer uso das competências que detém, colocando-as em prática e contribuir para a melhoria da qualidade dos cuidados de Enfermagem de Reabilitação na promoção da saúde. O objetivo do estudo foi descrever as relações entre funcionalidade, aptidão física e Qualidade de Vida (QdV) da população idosa da região Minho-Lima. Estudo quantitativo, do tipo descritivo-correlacional, numa amostra de 155 idosos de ambos os sexos, com idade igual ou superior a 70 anos da região Minho-Lima, por amostragem não probabilística de conveniência. Todos os idosos foram avaliados quanto à QdV (SF-36v2), funcionalidade (Índice de Barthel e Índice Lawton-Brody), aptidão física nas componentes de força muscular (testes Handgrip e Isometric Knee Extension) e capacidade cardiorrespiratória (teste dos seis minutos a andar). Para a análise dos resultados recorremos ao Statistical Package for the Social Sciences (SPSS), versão 19.0; o nível de significância para rejeitar a hipótese nula foi fixado em (α) ≤ 0,05. A média de idades é 78,9 anos. 71% sexo feminino e 29% sexo masculino. 63,2% pratica atividade física e 36,8% não. A média do Índice de Barthel situa-se em 92,42. O Índice Lawton-Brody apresenta média de 10,08. Os idosos avaliados que praticam atividade física atingem valores significativamente mais elevados no Índice de Barthel t(85,63)=3,12, p=0,001 e no Índice Lawton-Brody t(153)=2,72, p=0,007; apresentam melhor desempenho no teste de força muscular handgrip esquerda t(97,39)=2,23, p=0,029; exibem melhores níveis de QdV, obtendo valores significativamente mais elevados nos domínios da Saúde Geral t(153)=3,19, p=0,002, Desempenho Físico t(90,58)=2,49, p=0,015 Função Física t(153)=4,19, p=0,001 e Vitalidade t(153)=2,56, p=0,011. Os resultados mostram que quanto mais elevados são o Índice de Barthel e o Índice Lawton-Brody mais elevada é a QdV (r=0,22 a 0,61, p≤0,01). Verificamos ainda que a força de preensão manual esquerda, a força isométrica da perna direita e a capacidade cardiorrespiratória correlacionam-se de forma significativa com domínios da QdV - handgrip esquerda com Função Física (r=-0,16, p≤0,01), força isométrica perna direita com Desempenho Emocional (r=-0,30, p≤ 0,05), capacidade respiratória aos seis minutos com Função Física (r=0,16, p≤0,01), com Função Social (r=0,18, p≤0,01) e com Vitalidade (r=0,20, p≤0,01). A força de preensão manual esquerda correlaciona-se com o Índice de Barthel (r=-0,17, p≤0,01) e a capacidade cardiorrespiratória correlaciona-se com o Índice de Barthel (r=0,21, p≤0,05) e Índice Lawton-Brody (r=0,25, p≤0,05).A avaliação funcional efetuada pelo enfermeiro especialista em Enfermagem de Reabilitação pode contribuir para identificação das necessidades, promoção da saúde, prevenção de complicações, e manutenção ou aquisição de estilos de vida saudáveis, permitindo direcionar e fundamentar as ações de enfermagem para melhorar a capacidade funcional.
Given the aging, the current elderly population health needs represent an opportunity for the specialist nurse in Rehabilitation Nursing to make use of the powers he/she holds, putting them into practice and contributing to improve the quality of Rehabilitation Nursing care in health promotion. The aim of the study was to describe the relationship between functionality, physical fitness and quality of life (QoL) of the elderly population of Minho-Lima region. Quantitative study, descriptive and correlational type, in a sample of 155 elderly men and women, aged 70 or over 70 years of Minho-Lima region, by a non-probability sample of convenience. All of them were assessed for QoL (SF-36v2), functionality (Barthel Index and Lawton-Brody Index), physical fitness in muscle strength components (Handgrip tests and Isometric Knee Extension) and cardiorespiratory fitness (six minutes floor test). To analyze the results we used the Statistical Package for Social Sciences (SPSS), version 19.0; The significance level to reject the null hypothesis was set at (α) ≤ 0,05. The average age is 78,9 years. 71% female and 29% male. 63.2% practises physical activity and 36,8 % doesn’t. The mean of the Barthel Index stands at 92,42. The Lawton-Brody Index shows average of 10,08. The evaluated elderly who practise physical activity reach significantly higher values in Barthel Index t(85,63)=3,12, p=0,001 and Lawton-Brody Index t(153)=2,72, p=0,007; perform better in muscle strength handgrip left test t(97,39)=2,23, p=0,029; exhibit higher levels of QoL, obtaining significantly higher points in the fields of General Health t(153)=3,19, p=0,002, Physical Performance t(90,58)=2,49, p=0,015, Physical Function t(153)=4,19, p=0,001 and Vitality t(153)=2,56, p=0,011. The results show that the higher are the Barthel index and the Brody Lawton index, the higher is the QoL (r=0,22 a 0,61, p≤0,01). We also found that the strength of the left handgrip, isometric strength of the right leg and the cardiorespiratory capacity correlate significantly with domains of QoL - handgrip left with physical function (r=-0,16, p≤0,01), right leg isometric strength with Emotional Performance (r=-0,30, p≤0,05), respiratory capacity after six minutes with Physical function (r=0,16, p≤0,01), with Social Function (r=0,18 , p≤0,01) and Vitality (r=0,20, p≤0,01 ). The left hand grip strength correlates with the Barthel Index (r=-0,17, p≤0,01) and the cardiorespiratory fitness is correlated with the Barthel Index (r=0,21, p≤0,05) and Lawton-Brody Index (r=0,25, p≤0,05). The Functional assessment done by the specialist nurse in Rehabilitation Nursing can help to identify the needs, health promotion, prevention of complications, and maintain or acquire healthy lifestyles, allowing to direct and support the nursing actions to improve the functional capacity.
