Title: Capacidades e necessidades de pessoas idosas a residir em casa: Um estudo de base comunitária no Município de Esposende

Abstract: Um dos traços mais evidentes da sociedade portuguesa atual é a tendência para o aumento da longevidade e envelhecimento populacional (Paúl & Fonseca 2005). Porém, de acordo com Bastos, Faria e Lamela (2011) o problema atual é: (1) saber que semelhanças e diferenças existem entre os idosos (variabilidade intra e interpessoal), e (2) como contribuir para o bem-estar desses adultos mais velhos. Neste sentido, para Paúl (2005a) é importante não só estudar o indivíduo, mas também as políticas sociais e o planeamento dos serviços para idosos. Para esta autora, o ambiente é o grande desafio, sobretudo numa fase da vida em que as intervenções na pessoa são menos eficazes e a melhoria da qualidade de vida passa por mudanças ambientais. É neste contexto que a Gerontologia Social se torna pertinente, ao estudar o impacto das condições socioculturais e ambientais no processo de envelhecimento e na velhice, as consequências sociais desse processo, assim como as ações sociais que podem contribuir para otimizar o processo de envelhecimento (Fernández-Ballesteros, 2004). Face ao exposto, o presente estudo tem como objetivos: (1) descrever a população em estudo em função das características sociodemográficas, capacidades funcionais, cognitivas e socio-emocionais, assim como a utilização e necessidade sentida de serviços; (2) analisar as relações entre as variáveis em estudo. Tomando como referência os dados do Censo à população, o presente estudo conta com uma amostra de 84 pessoas idosas (cerca de 5% da população com 65 ou mais anos), a residir nas freguesias do Município sob investigação. Em termos de estratégias de amostragem foi utilizada a técnica “bola de neve”. A recolha de dados foi efetuada no domicílio, utilizando o Protocolo de Avaliação de Necessidades Comunitárias associadas ao Envelhecimento da População (ANCEP_GeroSoc; Bastos, Faria, Moreira, & Melo de Carvalho, 2011). Os dados foram analisados seguindo a tradição da investigação no domínio. Fazem parte deste estudo 84 pessoas idosas, maioritariamente do sexo feminino (61.9%), com uma média de idade de 74 anos (dp=6.57), a viverem em áreas predominantemente urbanas (54.8%), casadas (54.8%), com filhos (86.9%) e com níveis de escolaridade reduzidos (23.0% analfabetos e 57.0% com 1 a 4 anos de escolaridade). Todos os participantes partilham da condição de reformado. Estes idosos contam com uma rede de suporte social extensa, constituída por familiares, vizinhos e amigos. No que respeita aos serviços, dos cinco grupos avaliados, o grupo dos serviços sociais e recreativos (que inclui as viagens, o desporto e os serviços sociais/recreativos) é o que apresenta em termos globais maior utilização e necessidade sentida. A análise da relação entre variáveis indica que: (1) quanto mais independentes os idosos são nas atividades básicas da vida diária, mais independentes se mostram nas atividades instrumentais da vida diária, menos défice cognitivo e menos risco de sintomatologia depressiva apresentam; (2) quanto mais dependentes são nas atividades instrumentais da vida diária, mais risco de depressão apresentam; (3) quanto menor é o défice cognitivo, menor é a sintomatologia depressiva observada. A satisfação do idoso com a relação familiar não se mostra significativamente correlacionada com nenhuma das restantes dimensões do envelhecimento. Assim, a evidência reunida poderá ser tomada como ponto de partida para o desenho de futuras políticas, serviços e/ou programas de base comunitária com vista a promover a qualidade de vida e bem-estar das pessoas idosas.
One of the most evident traces of the current Portuguese society is the tendency for the increasing of longevity and the aging of population (Paúl & Fonseca 2005). However, according to Bastos, Faria and Lamela (2011), the current problem is: (1) to know which similarities and differences exist among the old people (intra and interpersonal variability), and (2) how to contribute to the well being of those aged adults. In this way, to Paúl (2005a) it is important not only to study the individual, but also the social politics and the planning of the services to the old people. To this author, the environment is the huge challenge, particularly in a phase of life in which the interventions in the person are less efficient and the improvement of the quality of life implies environmental changes. It is in this context that Social Gerontology becomes pertinent, by studying the impact of the socio-cultural and environmental conditions on the process of aging and in the old age, the social consequences of that process, as well as the social actions which may contribute to optimize the process of aging (Fernández-Ballesteros, 2004). Pertaining to what has been exposed, the present study has as aims: (1) to describe the population in study according to the sociodemographical characteristics, functional, cognitive and socio-emotional capabilities, as well as the use of services and the need felt for it; (2) analyze the relations between the variables in study. Taking as reference the data obtained from the Censes to the population, the current study relies on a sample composed by 84 old people (about 5% of the population with or more than 65 years old), residing on villages belonging to the municipality under investigation. In terms of strategy of sampling it was employed the “snowball” technique. The collecting of data was carried out in the domicile, using the Protocol of Evaluation of Communitarian Needs associated to the Aging of the Population (ANCEP_GeroSoc; Bastos, Faria, Moreira, & Melo de Carvalho, 2011). The data were analyzed following the tradition of investigation in this domain. In this study take part 84 old people, mainly of the feminine gender (61.9%), with an age average of 74 years old (dp=6.57), living in predominantly urban areas, (54.8%), married (54.8%), with children (86.9%) and with low levels of schooling (23.0% illiterate and 57.0% with 1 to 4 years of schooling). All the participants share the condition of retired. These old people count on an extensive network of social support, composed by relatives, neighbours and friends. In what to the services is concerned, of the five groups evaluated, the group of the social and recreational services (which includes travels, sports and the social/recreational services) is the one that presents, in a global way, higher utilization and the felt of necessity for it. The analysis of the correlation between variables indicates that: (1) the more independent the old people are with their basic activities of daily life, the more independent they show themselves in instrumental activities of daily life, presenting a lower cognitive deficit and less risks of depressive symptomatology; (2) the higher is their dependency in instrumental activities of daily life, the higher is the risk of depression which they present; (3) the lower is the cognitive deficit, the lower is the depressive symptomatology observed. The satisfaction of the old age with the family relationship does not appear to be significantly correlated with any of the other dimensions from aging. Therefore, the collected evidence can be taken as the starting point for the trace of future policies, services and/or communitarian based programs with the aim of promoting the quality of life and the well being of the old people.
