Title: Adesão à bundle cirúrgica em mulheres submetidas a histerectomia abdominal

Abstract: Os profissionais de saúde desempenham um papel fundamental na melhoria da qualidade dos cuidados. Contudo, podem também apresentar-se como agentes de transmissão e propagação de infeções. Sendo os enfermeiros dos profissionais que mais horas de cuidados prestam aos doentes, torna-se crucial o seu papel enquanto agentes dinamizadores na prevenção e controlo de infeção. Compete ao Enfermeiro Especialista identificar e implementar estratégias, em articulação com as várias estruturas das instituições, no sentido da prevenção da infeção do local cirúrgico (ILC). Há evidência da sua diminuição quando há envolvimento das chefias, das equipas multidisciplinares, dos profissionais de saúde, dos próprios doentes e a formação específica na adesão à Bundle cirúrgica/feixes de intervenção. Com o objetivo de avaliar a adesão à Bundle cirúrgica/feixes de intervenção cirúrgica dos enfermeiros pré, intra e pós- -operatórios, em mulheres submetidas a histerectomia abdominal, foi desenvolvido um estudo descritivo num hospital central no norte de Portugal, nos Serviços de Admissão Centralizada, Bloco Operatório (BO) e Internamento. A amostra foi constituída por 48 mulheres submetidas a histerectomia abdominal. O instrumento de colheita de dados foi uma check list e a análise de registos do processo clínico. Os resultados revelaram adesão total no banho pré-cirúrgico no dia da cirurgia com clorohexidina 2%. Já o banho da véspera nem sempre foi realizado (14,6%). Quanto à tricotomia, verificou-se algumas discrepâncias no método aplicado, tendo sido utilizadas as lâminas tipo gilete em 25,5% das mulheres, com a sua realização na véspera da cirurgia. A avaliação da glicemia capilar, maioritariamente, foi avaliada em mulheres com antecedentes em patologia diabética. No intraoperatório a administração do antibiótico (AB) profilático teve uma adesão de 100% e os tempos corretos para a sua administração foram de 91,7%. A adesão na avaliação da temperatura corporal foi de 35,4% no início e 31,3% no fim da cirurgia. Também a avaliação da glicemia capilar foi de 22,9% no início e 8,3% no fim da cirurgia. Quanto ao pós-operatório, verificou-se que na unidade de cuidados pós-anestésicos (UCPA) o AB profilático não se administrou em 97,9% e a monitorização da glicemia capilar foi realizada em 18,8% dos casos. No internamento, nas 24 horas após a cirurgia, verificou-se que a administração da profilaxia do AB foi mantida em 68,8% dos casos e a avaliação da glicemia capilar foi efetuada em 10,4%. Em síntese, a adesão à Bundle cirúrgica é elevada no banho pré-cirúrgico, tricotomia e antibioterapia, sendo menor nos restantes procedimentos, tornando-se necessário otimizar a adesão às intervenções preconizadas.
Health professionals play a crucial role on the improvement of quality in the care provided. However, they can also be responsible for the transmission and spread of infections. Being the Nurses one the professionals who spend more time caring for patients, they have a crucial role in the prevention and control of infection. It is the Specialist Nurse’s duty to identify and implement strategies, in collaboration with each institution, in order to prevent Surgical Site Infection (SSI). There is evidence that collaboration between leaders, managers, multidisciplinary teams, health professionals and patients themselves, along with specific training in adherence to Surgical Bundle/Bundles of Surgical Intervention, helps reduce SSI. With the aim of assessing implementation of Surgical Bundles/Bundles of Surgical Intervention by nurses in the pre, intra and post-operative, in women undergoing abdominal hysterectomy, it was carried out a descriptive study in a hospital in northern Portugal, in the central Admission Services, Operating Rooms and Internment Services. The study consisted in 48 women submitted to abdominal hysterectomy. The method used to collect data was a check list and a consultation of the clinical process. The results revealed full implementation on the pre-surgical bath on the day of surgery with 2% chlorhexidine. However, the bath one day prior to the surgery was not always performed (14.6%). As for the hair removal, there were some discrepancies in the method used, as razor blades were used in 25.5% of the cases, by the women, on the eve of the surgery. The glycemic control was mostly evaluated in women with a history of diabetic pathology. In the intraoperative period the administration of prophylactic AB had a 100% adherence rate. However, for the correct timing of administration there was an adherence rate of 91.7%. As for the body temperature assessment there was only a 35.4% adherence at the start and 31.3% at the end of surgery. The glycemic control was also 22.9% at the start and 8.3% at the end of surgery. Regarding the postoperative period, it was verified that in the UCPA the prophylactic AB was suspended in 97.9% of the cases and the glycemic control monitoring was performed in only 18.8% of the cases. As for the hospital interment, it was verified that AB prophylaxis was maintained in 68.8% of the cases and the evaluation of glycemic control was performed in 10.4%. In conclusion, the adhesion to the Bundle is higher in the pre-surgical bath, trichotomy and antibiotic therapy, and lower in the other procedures, making it necessary to optimize adherence to all the recommended interventions.
