dc.description.abstractA gravidez surge como um processo fisiológico natural, que envolve uma sequência de adaptações no organismo materno, inerentes ao desenvolvimento fetal e crescimento do bebé, processo que se inicia logo nas primeiras semanas após a concepção. Embora o organismo materno esteja naturalmente preparado, o processo envolve ajustes anatómicos e fisiológicos. Algumas tarefas da vida diária começam a tornar-se difíceis de realizar, pelas grávidas, como por exemplo o andar, o alcançar objectos, o ficar em posição erecta estática, o sentar, o deitar, o subir escadas, o transportar objectos, entre outros. (Konkler & Kisner, 1996). Neste sentido, o equilíbrio postural nesta população, tem sido um desafio de investigação de vários autores. A literatura tem revelado que a estabilidade postural diminui durante a gravidez, principalmente no 2º e 3º trimestres de gravidez (Butler, Colón, Druzin, & Rose, 2006; Mann, Kleinpaul, Teixeira, & Mota, 2011; Nagai et al., 2009), tanto para a condição de olhos abertos como para a condição de olhos fechados (Butler et al., 2006). Quando a dificuldade da tarefa aumenta (exemplo da superfície instável), a visão é de extrema importância para o controlo postural no desenvolvimento de tarefas, mesmo quando os sistemas proprioceptivos e vestibulares estão presentes (Mann et al., 2011; Oliveira, Vieira, Macedo, Simpson, & Nadal, 2009). Na grande maioria dos estudos de avaliação do equilíbrio postural, o instrumento mais utilizado é a plataforma de forças, que diverge de estudo para estudo quanto à sua marca e quanto às metodologias/condições de realização dos testes. A posturografia computadorizada – Neurocom International, Inc. tem sido recomendada para a avaliação do equilíbrio postural, onde alguns estudos (aplicados a grávidas e a idosos) utilizam a plataforma Equitest (Camicioli, Panzer, & Kaye, 1997; McCrory, Chambers, Daftary, & Redfern, 2011; Wrisley & Whitney, 2004; Zammit, Wang-Weigand, & Peng, 2008) e outros (aplicados a idosos), utilizam a plataforma Balance Master (Brech, Andrusaitis, Vitale, & Greve, 2012; Jbabdi, Boissy, & Hamel, 2008; Tomomitsu, Alonso, Morimoto, Bobbio, & Greve, 2013). São poucos os estudos que utilizam este instrumento, plataforma Balance Master, para avaliar o equilíbrio postural em grávidas. Assim sendo, o principal objectivo do nosso estudo foi perceber a capacidade da plataforma Balance Master – Neurocom International Inc. de avaliar e detectar as diferenças no controlo postural entre as mulheres grávidas do 2º e 3º trimestres de gravidez. Pretendemos, também com este estudo, verificar as diferenças do controlo postural, entre grávidas, nas condições olhos abertos/olhos fechados, superfície firme/superfície instável, na subida/descida do degrau, assim como, na exploração dos limites de estabilidade. Para dar resposta aos objectivos definidos, foram recrutadas 7 grávidas no 2º trimestre de gravidez e 5 grávidas no 3º trimestre de gravidez, todas residentes na região do Alto Minho (distrito de Viana do Castelo). Para a avaliação do equilíbrio postural foram seleccionados os testes: modified CTSIB (olhos abertos/olhos fechados; superfície firme/superfície instável), limits of stability e o teste step up/over. Da análise dos resultados podemos perceber que existe uma diminuição do equilíbrio postural para a condição de olhos fechados/superfície instável nas grávidas do 2º e 3º trimestres de gravidez, corroborando outros estudos, onde a visão e a superfície instável mostraram afectar o equilíbrio postural em grávidas (Oliveira et al., 2009; Shailesh & Champa, 2001). Comparados os grupos, verificamos que existem diferenças significativas para a condição olhos abertos/superfície instável, revelando que o grupo do 3º trimestre de gravidez apresenta maior estabilidade postural, realçando a importância da informação visual. Tudo indica que sem o transporte da mala de senhora, as grávidas apresentam maior instabilidade. Não foram encontradas diferenças significativas entre os grupos no teste de subida/descida do degrau. Quando avaliados os limites de estabilidade, os resultados revelam existirem diferenças significativas entre os grupos, indicando que a velocidade de movimento das grávidas para trás é significativamente menor nas grávidas do 3º trimestre de gravidez. No 3º trimestre as grávidas também parecem ser mais cautelosas na velocidade dos seus movimentos, talvez pelo receio de perderem o equilíbrio e como consequência provocar quedas. Os resultados apontam ainda para uma correlação negativa entre a estatura e a velocidade de deslocamento do centro de pressão-olhos abertos e superfície firme e a velocidade de deslocamento do centro de pressão-olhos abertos e superfície instável, sugerindo que as mulheres grávidas mais altas, em situações de presença de informação visual, apresentam menor velocidade de deslocamento do centro de gravidade, quer em superfícies firmes quer em superfícies instáveis. Podemos concluir que, apesar da limitação do tamanho da amostra, a plataforma Balance Master apresenta algumas limitações na avaliação do equilíbrio postural em mulheres grávidas, dado não permitir uma avaliação individualizada relativamente ao eixo antero-porterior e medial-lateral. Contudo torna-se pertinente na avaliação do equilíbrio postural em situações representativas da vida diária da grávida. Como futuras investigações sugerimos a realização de estudos de avaliação do equilíbrio postural com a plataforma Balance Master, onde a amostra seja maior, para que os resultados tenham mais consistência, assim como considerar a possibilidade de usar a técnica do match pair para poder definir um grupo de controle. Seria interessante a realização de um estudo longitudinal, onde o objectivo se centrasse na avaliação do equilíbrio postural ao longo da gravidez (1º, 2º e 3º trimestres de gravidez) assim como no período pós-parto. Observar a influência da prática de actividade física no equilíbrio postural em grávidas do 2º e do 3º trimestres de gravidez e no pós-parto, seria também um interessante tema de investigação.pt_PT
dc.description.abstractPregnancy comes as a natural physiological process that involves a sequence of adaptations in the maternal organism, inherent to the fetal development and growth of the baby, a process that begins in the first weeks after conception. Although the maternal organism is naturally prepared, the adjustment process involves anatomical and physiological aspects. Some tasks of daily life begin to become difficult to realize, by pregnant women, such as walking, to reach objects, stay in static upright position, sit, lay in the bed, climbing stairs, carrying the objects, among others (Konkler & Kisner, 1996). In this regard, postural balance in this population has been a challenge for research of several authors. Literature has revealed that postural stability decreases during pregnancy, especially in the 2nd and 3rd trimesters of pregnancy (Butler et al., 2006; Mann et al., 2011; Nagai et al., 2009), both for the open eyes condition as for the closed eyes condition (Butler et al., 2006). When the task difficulty increases (eg the unstable surface), vision is of utmost importance for the postural control in the development of tasks, even when the proprioceptive and vestibular systems are present (Mann et al., 2011; Oliveira et al., 2009) In the majority of evaluation studies of postural balance, the most used instrument is the force platform, which differs from study to study as to its brand and the methodologies/conditions under which the tests are performed. The computerized posturography - Neurocom International, Inc. has been recommended for the evaluation of postural balance, where some studies (applied to pregnant women and the elderly ) use the platform Equitest (Camicioli et al., 1997; McCrory et al., 2011; Wrisley & Whitney, 2004; Zammit et al., 2008) and others (applied to the elderly) , using the Balance Master platform (Brech et al., 2012; Jbabdi et al., 2008; Tomomitsu et al., 2013). There are just a few studies that use this instrument, the Balance Master, in postural balance in pregnancy research. Therefore, the main objective of our study was to understand the platform Balance Master - Neurocom International Inc. capacity to evaluate and detect the differences in postural control among pregnant women in the 2nd and 3rd trimesters of pregnancy. Intend, also in this study, to see the differences in postural control among pregnant women conditions eyes open/eyes closed and firm surface/unstable surface, up/down of the step, as well, as the exploitation of stability limits. To meet the defined objectives, were recruited seven pregnant women in the 2nd trimester and 5 pregnant in the 3rd trimester of pregnancy, all residents in the Alto Minho (district of Viana do Castelo). For the evaluation of postural balance were selected: modified CTSIB (eyes open / closed eyes, firm surface / unstable surface), limits of stability test and step up / over. Analysis of the results we can see that there is a decrease in postural balance for the closed eyes condition/unstable surface in pregnant women in the 2nd and 3rd trimesters of pregnancy, corroborating others studies, where vision and unstable surface affected postural balance in pregnant women (Oliveira et al., 2009; Shailesh & Champa,2001). Comparing the groups, we found significant differences in the eyes open/unstable surface condition, revealing that the 3rd trimester of pregnancy has greater postural stability, showing the importance of visual information. Everything indicates that without carrying lady's handbag, pregnant women have greater instability. No significant differences were found between groups in the test step up/over the steps. When assessing the limits of stability, the results reveal significant differences between groups, indicating that the backward movement speed is significantly lower in pregnant women in the 3nd trimester of pregnancy. In 3rd pregnant women appear to be more cautious with the speed of their movements, perhaps for fear of losing balance and cause a fall. The results also point to a negative correlation between height and the speed of the pressure center - eyes open/firm surface and the speed of the pressure center - eyes open/unstable surface, suggesting that taller pregnant women's, in situations of presence of visual information, have lower speed of displacement of the center of gravity, either in firm or unstable surfaces. We can conclude that, despite the limitations of the sample size, the Balance Master platform has some limitations in the assessment of postural balance in pregnant women cannot be thus considered the best tool for assessing postural balance, as the instrument is not prepared to assess centre of pressure on both anterior-posterior and medial-lateral axis. However, balance master demonstrates functionality in assessment of postural balance in situations representative of the daily life of the pregnant woman. As future research we suggest conducting studies assessing postural balance with the Balance Master platform, with a higher sample, so the results are more consistent, as well as consider the possibility of using the match pair technique to be able to define a group control. It would be interesting to conduct a longitudinal study, where the aim is focused on the evaluation of postural balance throughout pregnancy (1st, 2nd and 3rd trimesters of pregnancy) as well as in the postpartum period. Observe the influence of physical activity on postural balance in pregnant women in the 2nd and 3rd trimesters of pregnancy and postpartum would also be an interesting research topic.pt_PT
