Abstract: A doença renal crónica e o tratamento de substituição da função renal por hemodiálise, são responsáveis por uma série de alterações, que se manifestam na pessoa com insuficiência renal crónica terminal. Muitas destas alterações, desde as fisiológicas mais complexas, às psicológicas e sociais têm impacto negativo na sua funcionalidade. Torna-se por isso fundamental a implementação de programas especializados e individualizadas de reabilitação, de forma regular que contemplem atividades maximizadoras das capacidades funcionais, e o enfermeiro de reabilitação deve aplicar os seus conhecimentos e competências no cuidar das necessidades destas pessoas. Daí a opção de fundamentar a nossa prática com evidência científica e realizar um estudo com o objetivo de avaliar os efeitos de um programa de reabilitação funcional intradialítico, na pessoa com insuficiência renal crónica em programa regular de hemodialise. A nossa opção metodológica, para dar resposta a este objetivo foi desenvolver um estudo quase-experimental com dois grupos. O grupo intervenção que integrou participantes em programa de reabilitação funcional e o grupo de controlo com participantes que não integraram qualquer programa. Foram feitas duas avaliações aos participantes, uma antes e outra depois da intervenção. Recorremos a uma amostra por conveniência constituída por quarenta e sete pessoas, insuficientes renais em programa regular de hemodialise, que fazem tratamento numa Clinica de Hemodialise da zona norte. Vinte e quatro pessoas integraram o grupo intervenção e vinte e três o grupo controlo. Ao grupo de intervenção foi aplicado um programa de reabilitação funcional intradialítico, três vezes por semana durante doze semanas, composto por exercícios respiratórios de relaxamento com inspirações profundas e de aquecimento dos diferentes segmentos com alongamentos no início e fim da sessão e durante as sessões foram combinados exercícios dinâmicos, de contração isométrica e isotónica, resistidos e não resistidos. Foram efetuadas análises descritivas e inferenciais no tratamento estatístico dos dados. Os principais resultados revelam que o equilíbrio, a força muscular e a mobilidade foram sensíveis ao programa de reabilitação funcional, com ganhos na capacidade funcional, a composição corporal só foi sensível no que se refere à diminuição do tecido adiposo, não se verificando alterações no tecido magro nem no IMC.
Chronic renal dysfunction and the treatment by means of substitution of renal function for hemodialysis, are responsible for the series of alterations manifested in an individual with terminal chronic renal failure. Several of these changes, from the most complex physiological to psychological and social changes, have a negative impact on the functionality of an individual. This makes it fundamentally important the implementation of specialized and customised rehabilitation programs in a regular basis, that contemplate activities that will make the most of the functional capacities of these individuals. A rehabilitating nurse must apply their knowledge and competencies while caring for these patients. Therefor the need to solidify our practice with scientific evidence and prepare a study with the objective of revealing the effects of an intradyalitic functional rehabilitation program, in a patient with chronic renal insufficiency participating regularly in a hemodialysis program. In order for this we developed an almost-experimental study with two functional groups. The intervention group, consistent of participants that are in a functional rehabilitation program and the control group consistent of participants that are not a part of any program. Two evaluations were conducted on the participants, one before and one after the intervention. We used a convenient sample size of forty seven people, with kidney failure participating regularly in a hemodialysis program, being treated at a hemodialysis clinic in the north of Portugal. Twenty four individuals were a part of the intervention group, and 23 composed the control group. The intervention group was submitted to a functional intradyalitic rehabilitation program, throughout twelve weeks, three times a week. This program consisted of respiratory relaxing exercises, consisting of deep breaths, as well as warm up exercises that included stretches in the beginning and end of the sessions as well as dynamic exercises during the session, with isometric and isotonic contractions, resisted and not resisted. Descriptive and inferential statistical analysis was conducted on data. The major results revealed that the balance, the mobility and the muscular strength were sensitive to the functional rehabilitation program, with positive effects noted on the functional capacity. The corporal composition was only sensitive in regards to the reduction of adipose tissue in the intervention group. No altered evidence was noted in lean mass and in IMC.
