Cuidados paliativos além fronteiras: implementação de uma unidade de cuidados paliativos em França

Abstract

A inversão da pirâmide etária é uma realidade efetiva, presente nas últimas décadas em toda a Europa. Este fenómeno traz como repercussões, o aumento da população idosa e o consequente aumento das doenças crónicas, evolutivas e terminais, com impacto na organização dos sistemas de saúde e na constituição de recursos destinados a este tipo de doentes. Sob este prisma, os CP têm vindo assumir-se como uma abordagem que visa a prevenção do sofrimento multidimensional e que integram o processo de morrer na vida, tendo sempre como objetivo principal o bem-estar e qualidade de vida da pessoa doente e família, de forma a proteger a sua dignidade. O Conselho da Europa reconhece que existem deficiências no apoio/assistência a quem enfrenta a terminalidade da vida, recomendando uma maior atenção para as condições de vida de quem enfrenta esta última etapa da vida, e para a necessidade de se oferecer a estes doentes cuidados humanos em ambientes acolhedores que promovam a proteção da dignidade na fase final de vida. Como tal, é fundamental a existência de recursos específicos que deem resposta às necessidades globais do doente e família de forma efetiva através de uma abordagem multimodal. Assim, é fundamental que todo o cidadão tenha acesso aos cuidados paliativos e para isso torna-se necessário que haja mais estruturas em cuidados paliativos, quer seja em sistemas públicos ou privados. Neste sentido, o principal objetivo deste Trabalho de Projeto é a implementação de uma unidade de cuidados paliativos numa estrutura de saúde privada em Paris, de modo a dar resposta aos doentes com doença crónica, evolutiva e terminal da região. Para isto, utilizou-se a Metodologia de Projeto, uma vez que esta é uma metodologia centrada no estudo de problemas, com o intuito de contribuir para o desenvolvimento da prestação de cuidados paliativos com competência, assegurando a eficácia, eficiência a adequação a segurança, equidade e a satisfação. Para a concretização deste Trabalho de Projeto passamos por várias fases, tais como: diagnóstico de situação, definição dos objetivos, e planeamento da implementação da Unidade de Cuidados Paliativos.

The inversion of the age pyramid is an effective reality, present in the last decades throughout Europe. This phenomenon results in an increase in the elderly population and the consequent increase in chronic, evolutionary and terminal diseases, with an impact on the organization of health systems and the constitution of resources for this type of patient. In this light, the palliative care have become an approach that aims to prevent multidimensional suffering and that integrate the process of dying in life, always having as main objective the well-being and quality of life of the sick person and family, in order to protect their dignity. The Council of Europe recognizes that there are shortcomings in the support/care of those facing life termination by recommending greater attention to the living conditions of those facing the latter stage of life and to the need to provide these patients with human care in peaceful environments that promote the protection of dignity in the final stage of life. As such, it is crucial to have specific resources that effectively address the overall needs of the patient and family through a multimodal approach. Therefore, it’s fundamental that all citizens have access to palliative care and for this it’s necessary that there be more structures in palliative care, whether in public or private systems. In this sense, the main objective of this Project Work is the implementation of a palliative care unit in a private health structure in Paris, in order to respond to the needs of the patients with chronic, evolutionary and terminal disease in the region. To accomplish this, the Project Methodology was used, because this is a methodology focused on the study of problems, with the purpose of contributing to the development of the palliative care and favouring the effectiveness of the care provided. For the accomplishment of this Project Work we went through several phases, such as: diagnosis of situation, definition of objectives, and planning of the implementation of the Palliative Care Unit.
