A Perspetiva dos terapeutas da fala sobre a sua intervenção numa equipa de cuidados paliativos

Abstract

Os cuidados paliativos são cuidados globais e contínuos que pretendem assegurar o bem-estar físico, psicológico, social, económico e espiritual do doente. Este tipo de cuidados considera o doente e a família como uma unidade a cuidar centrando neles a intervenção, respeitando as suas preferências, objetivos e valores culturais, sociais e espirituais. Os cuidados paliativos devem reger-se por uma abordagem transdisciplinar, sendo que o ideal seria que as equipas integrassem uma variedade de profissionais, estando incluídos os médicos, enfermeiros, psicólogos, assistentes sociais e outros profissionais como é o caso do terapeuta da fala. Apesar de se conhecer a importância deste profissional para o bem-estar físico, mental e social do doente em fim de vida, em Portugal, a inclusão de terapeutas da fala nas equipas de cuidados paliativos é ainda muito diminuta. Para além disto, a formação dos terapeutas da fala ao nível dos cuidados paliativos é muito deficitária e são escassos os estudos direcionados para a inclusão destes profissionais em equipas de cuidados paliativos. Tendo como convicção que estes profissionais são necessários nas equipas de cuidados paliativos surge a pertinência social e cientifica desta investigação. O presente estudo tem como objetivo geral: Conhecer a perspetiva dos terapeutas da fala sobre a sua intervenção em equipas de cuidados paliativos. A partir de uma abordagem metodológica qualitativa enquadrada num estudo exploratório de nível I., a entrevista semi-diretiva foi aplicada junto de terapeutas da fala com o intuito de aprofundar conhecimentos sobre os significados que atribuem à sua intervenção e integração nos cuidados paliativos. Pretende-se alargar os conhecimentos dos terapeutas da fala e da equipa multidisciplinar dos cuidados paliativos, trilhando um caminho para a inclusão deste técnico em equipas de cuidados paliativos. Dos principais resultados obtidos destacamos que a integração do terapeuta da fala nas equipas de cuidados paliativos em Portugal é considerada importante, reconhecendo este profissional como especializado na intervenção das perturbações da comunicação e da disfagia. Apesar disto, continua a existir um grande desconhecimento das funções do terapeuta da fala e uma falta de recursos financeiros para a integração deste profissional no âmbito dos cuidados paliativos.

Palliative care is a global and continued care aimed at assuring the physical, physiological, social and spiritual well-being of a patient. This type of care treats the patient and the family as a unity to care, which is the target of the intervention, respecting their preferences, goals and cultural, social and spiritual values. Palliative care should have a transdisciplinary approach, ideally comprising a variety of professionals, including doctors, nurses, psychologists, social workers and other professionals, such as speechlanguage pathologists. Although the importance of these professionals for the physical, psychological and social well-being of the end-of-life patients, in Portugal, is well-known, there is still a low inclusion of them within the palliative care teams. Furthermore, the training of the speech-language pathologists to work in palliative care is very faulty and there are scarce studies regarding the inclusion of speech therapy in the palliative care. Having the conviction that these professionals are required in the palliative care teams, the social and scientific relevance of this study arises. The present work has the following general goal: knowing the speech-language pathologists perspective about their own intervention in the palliative care teams. Using a qualitative methodologic approach, comprised on a level I exploratory study, a semi-directive interview was applied to speech-language pathologists, aiming at increasing knowledge about the outcome of their intervention and integration on the palliative care. It is intended to broaden the knowledge of the speech-language pathologists and the palliative care multidisciplinary team, opening a way for the inclusion of this technician on the palliative care teams. Among the main results, it is highlighted that the integration of the speechlanguage pathologists on the palliative care teams in Portugal is considered to be important, with this professional being recognized as a specialized technician for the intervention in communication and dysphagia. Despite this, there is still a big lack of knowledge about the role of the speech-language pathologists and a lack of financial resources for the integration of speech-language pathologists in the field of palliative care.
