A intervenção do enfermeiro especialista em enfermagem de reabilitação na pessoa após AVC

Abstract

O Acidente Vascular Cerebral (AVC) é a principal causa de morte em pessoas acima de 60 anos, e a principal causa de incapacidade em todo o mundo, estima-se que ocorrem 30 novos AVC’s e destes morrem 6 pessoas a cada 60 segundos em todo o mundo, segundo a World Stroke Campaign (2014-2016) in Organização Mundial da Saúde (2018). O AVC é normalmente caraterizado por aparecimento súbito com sinais e sintomas, tais como: desvio da comissura labial; dificuldade em falar e falta de força num braço/perna (hemicorpo). A reabilitação deve iniciar-se entre as 48 e as 72 horas após o AVC, sendo que o papel do Enfermeiro Especialista em Enfermagem de Reabilitação é primordial na intervenção motora, sensorial, cognitivo, cardiorrespiratório, alimentação, eliminação e sexualidade, de forma a maximizar as capacidades funcionais (Ordem Enfermeiros, 2017). Como metodologia optou-se pelo Trabalho Projeto tendo como objetivo principal conhecer o nível de independência da pessoa após o AVC para o autocuidado (Alimentação; Higiene Pessoal; Controlo da eliminação vesical e intestinal e uso de sanitário; Vestuário e Locomoção/Transferência), fazendo a ponte entre a teoria que posteriormente será aplicado na prática, com o intuito de resolver os problemas detetados. De forma a realizar um diagnóstico de situação foi selecionado como instrumento para a recolha de dados a entrevista semiestruturada, através da qual foi colocado um conjunto de questões previamente determinadas e direcionadas para o objetivo do trabalho. Também, recorremos à análise SWOT, onde foi necessário fazer uma pesquisa aprofundada dos pontos fortes e fracos em ambiente interno e identificar as oportunidades e ameaças em ambiente externo. Participaram 10 pessoas que sofreram AVC e seus cuidadores, convocados para a consulta externa de Medicina Interna - AVC, numa Unidade Local de Saúde da região norte do país. Após o diagnóstico de situação pudemos concluir que 90% da nossa amostra ficou com alguma limitação após AVC, apresentando na sua grande maioria falta de independência nos autocuidados e falta de informação aumentando as dificuldades em arranjar estratégias em ambiente domiciliar. Em paralelo percebemos a importância de envolver o cuidador neste processo de transição saúde-doença. Para resolução dos problemas identificados propomos a criação de uma Consulta de Enfermagem à pessoa após AVC e cuidador, tendo como objetivo geral a promoção da independência para o autocuidado e reintegração familiar e social. Para o efeito foram definidos objetivos específicos e efetuado o planeamento de atividades de forma a permitir uma resposta personalizada aos problemas apresentados pela pessoa após AVC.

The Stroke (AVC) is the leading cause of death in people over 60, and the leading cause of disability worldwide. It is estimated that 30 setrokes occur and out of these, 6 people die every 60 seconds throughout the world, according to the World Stroke Campaign (2014- 2016) in World Health Organisation (2018). The stroke is usually characterized by sudden onset of signs and symptoms, such as: deviation of the labial commissure; difficulty in speaking and lack of strength in an arm / leg (hemibody). Rehabilitation should begin between 48 and 72 hours after stroke, and the role of the Nurse Specialist in Rehabilitation Nursing is essential when dealing with rehabilitation of motor, sensory, cognitive, cardiorespiratory, feeding, sexual and elimination abilities, in order to maximize its functional capacities (the Nursing Council, 2017). We opted for Work Project as methodology to carry out this study. The main objective of this project was to know the level of independence of the person after the stroke when it comes to self-care (food, personal hygiene, control of bladder and intestinal elimination and sanitary use, clothing and locomotion / transference) providing a bridge between the theory and the practice that will later be applied, in order to solve the problems detected. To carry out a diagnosis of the situation, a semi-structured interview was selected as an instrument for collecting data, through which a set of previously determined questions were assigned to the work objective. We also used the SWOT analysis, where it was necessary to do an in-depth research of the strengths and weaknesses in the internal environment and identify the opportunities and threats in the external environment. Ten people who suffered stroke and their caregivers were invited to the outpatient appointment of Internal Medicine (AVC) at a Local Health Unit in the northern region of the country. After the diagnosis of the situation, we could conclude that 90% of our sample had some limitation after stroke, most of them lack of self-care independence and lack of information, increasing the difficulties in arranging strategies to apply in the home environment. In parallel, we perceive the importance of involving the caregiver in this process of health-toillness transition. To solve the identified problems we propose the creation of a Nursing Rehabilitation Consultation for the person after stroke and caregiver, with the general objective of promoting independence for self-care and family and social reintegration. For this purpose, specific objectives were defined and activities were planned in order to allow a personalized response to the problems presented by the person after stroke.
