Reeducação funcional respiratória pré-operatória na pessoa submetida a cirurgia abdominal

Abstract

A toda atividade cirúrgica estão inerentes complicações, sendo estas, eventos adversos que influenciam negativamente o percurso normal de recuperação da pessoa cirúrgica. A pessoa é referida como pessoa cirúrgica a partir do momento que lhe é realizado um diagnostico do foro cirúrgico. A cirurgia abdominal é um procedimento que envolve a cavidade abdominal e neste tipo de cirurgia as complicações respiratórias são das mais frequentes, que acarretam morbilidade e mortalidade perioperatória, contribuindo para o aumento do tempo de internamento e reinternamentos e consequentemente maiores gastos para os serviços de saúde. A eficácia dos programas de reeducação funcional respiratória está demostrada na literatura, quer na prevenção de complicações respiratórias pós-operatórias, quer no seu tratamento e redução da gravidade nos procedimentos que envolvem a cavidade abdominal. Estes devem ser iniciados o mais precocemente possível, no período pré-operatório e sustentados ao longo do período pós-operatório. O enfermeiro especialista em enfermagem de reabilitação esta dotado de competência humana, científica e técnica que lhe permite intervir em todo o percurso da pessoa cirúrgica, com o objetivo de prevenir alterações da função respiratória, mas também da sua recuperação através da reeducação funcional respiratória, sendo esta uma vasta área de intervenção da enfermagem de reabilitação. Na busca da melhoria da qualidade dos cuidados em saúde e na evidencia científica, desenvolvemos um estudo e optamos pela metodologia de trabalho projeto. No diagnostico de situação, utilizamos a entrevista semiestruturada realizada a seis pessoas no pré-operatória de cirurgia abdominal e concluímos que no pré-operatório não são dotados de conhecimentos nem habilidades sobre exercícios respiratórios, mas consideram importantes e úteis. Recorremos, ainda, à análise SWOT onde foram identificamos vários pontos fortes. No sentido de dar resposta ao identificado, propomos a criação de uma consulta de enfermagem pré-operatória de reeducação funcional respiratória, tendo como objetivos: Capacitar a pessoa submetida a cirurgia abdominal para a execução de exercícios respiratórios no pré e pós-operatório e prevenir complicações respiratórias na pessoa submetida a cirurgia abdominal. Procedemos ao planeamento de sessões que consideramos dar resposta aos objetivos atendendo a que vão de encontro às necessidades identificadas. Apresentamos, ainda, proposta de avaliação do projeto, recorrendo a indicadores capazes de traduzir os ganhos em saúde, através dos contributos dos cuidados de enfermagem especializados em enfermagem de reabilitação. A etapa de execução não foi operacionalizada pelo facto do projeto ainda não estar aprovado.

To each and every surgical activity there are inherent certain complications, which are adverse events that negatively influence the normal course of recovery of the surgical person. The person is referred to as surgical person from the moment he is diagnosed in surgical forum. The abdominal surgery is a procedure that covers the abdominal cavity, and in this type of surgery respiratory complications are the most common, which result in perioperative morbidity and mortality, contributing to an increase of the internment and reinternment period and consequently more expenses for the health services. The effectiveness of the functional reeducation programs is supported in literature, both in the prevention of postoperative respiratory complications and in its treatment and reduction of severity in procedures involving the abdominal cavity. These should be started as early as possible in the preoperative period and sustained throughout the postoperative period. The specialist nurse in rehabilitation nursing is endowed with human, scientific and technical competence that allows him to intervene throughout the surgical person course, with the aim of preventing changes in respiratory function, but also of recovery through respiratory functional reeducation, this is a wide area of intervention for rehabilitation nursing. In the search for the improvement of health care quality and scientific evidence, we developed a study and we chose for the project work methodology. In the situation diagnosis, we used the semi-structured interview performed to six people in the preoperative period of abdominal surgery and we concluded that in the preoperative period they are not endowed with knowledge or skills about breathing exercises, but considered important and useful. We also used the SWOT analysis where several strengths were identified. In order to respond to the identified, we suggest the creation of preoperative nursing appointment for functional respiratory re-education, with the following goals: to enable the person undergoing abdominal surgery to perform respiratory exercises before and after surgery and to prevent complications in the person undergoing abdominal surgery. We proceed to the planning of sessions that we consider to respond to the goals taking into account that they meet the needs identified. We also present a project evaluation proposal, using indicators capable of translating health gains, through the contributions of specialized nursing care in rehabilitation nursing. The implementation stage was not operationalized because the project was not yet approved.
