Title: Envelhecimento e Eutanásia: um estudo sobre representações sociais de estudantes da pré e pós-graduação em Gerontologia

Abstract: Contexto e objetivo. O envelhecimento é um processo multidimensional e progressivo de mudanças a nível biológico, psicológico e social, composto por ganhos e perdas que ocorrem ao longo da vida (WHO, 1999). A longevidade refere-se à duração da vida de um organismo e depende da progressão do envelhecimento, pois este impõe-lhe um limite mais tarde ou mais cedo (i.e. o número de anos a viver). Segundo Zaterka (2015) há diferenças entre a longevidade e a saúde, a quantidade e a qualidade de vida, ou seja, entre o adiantamento da morte (por meios de regimes e drogas) e um prolongamento efetivo da vida. Precisamos de refletir até que ponto devemos prolongar essa vida e quais são as suas consequências, sendo necessário ter em atenção os limites do prolongamento indefinido da vida, quando não há esperança na recuperação do moribundo, em que o melhor é uma morte fácil e justa. A morte diz respeito à cessação definitiva da vida, sendo que morrer é parte integrante da vida, tão natural e previsível como nascer (Horta, 1999). A perceção da morte é distinta para cada um de nós, quer pelas suas diversas representações, conceções e crenças, sendo fundamentadas não só na sua ocorrência, mas também no contexto social e familiar em que o indivíduo esteja inserido (Gomes, 2013). A eutanásia, sendo uma prática de morte assistida, tem como significado uma boa morte, uma morte sem sofrimento e sem dor (Demarchi & Bastos, 2013). Segundo Suski (2012), a eutanásia passou a ser um ato de liberdade em que a pessoa que está com uma doença terminal e incurável, verificando-se que esta está perto da morte, considerando que a sua vida não tem qualidade nem dignidade, há a possibilidade de ser reconhecido o direito à pessoa de decidir pôr fim à sua vida. O suicídio medicamente assistido consiste na ajuda solicitada pelo doente, para que outro o auxilie a executar a sua morte. As descrições da eutanásia são construções culturais que evoluíram ao longo dos anos, dependendo do ambiente cultural e social (Jylhänkangas, Smets, Cohen, Utriainen & Deliens, 2014). Neste contexto, o presente estudo tem como objetivo identificar as representações sociais da eutanásia e outros aspetos do fim de vida em estudantes dos cursos do âmbito do envelhecimento. Método. Neste estudo qualitativo exploratório, participaram 75 estudantes, sendo 53 estudantes da licenciatura em Educação Social Gerontológica e 12 estudantes do mestrado em Gerontologia Social. A recolha de dados foi realizada com um questionário composto por 12 questões abertas, visando explorar aspetos associados à morte, eutanásia e o suicídio medicamente assistido. Quanto à estratégia de análise de dados procedeu-se à análise de conteúdo, mais concretamente à pré-análise (organização do material), exploração do material (codificação do material, classificação e categorização da informação) e tratamento dos dados (interpretação da informação). Resultados. No presente estudo a taxa de participação foi de 76%, face ao número de inscritos (n=99), sendo que a maioria dos participantes é do género feminino (97%). A análise de dados permitiu identificar três grandes temas designadamente: eutanásia, morte e suicídio medicamente assistido. No que diz respeito à morte foi possível identificar a morte como acontecimento de vida normativo, sendo um acontecimento predominantemente encarado de modo negativo; 48% dos participantes pensa sobre a sua própria morte e 71% dos participantes pensa sobre a morte dos familiares e amigos. Quanto à eutanásia, para os participantes esta diz respeito a uma opção voluntária e/ou consentida de pôr fim à vida, cerca de 59% dos participantes é favorável a este procedimento, 44% dos participantes, enquanto familiar favorável à eutanásia em situação de doença incurável e em sofrimento, aceitam a decisão do familiar/doente, 45,33% dos participantes enquanto doente em situação de doença incurável e em sofrimento optava pela eutanásia, 53,34% dos participantes considera a eutanásia um tema tabu e 65,34% dos participantes é claramente a favor da legalização. Quanto ao suicídio medicamente assistido para os participantes este diz respeito á ação do próprio que toma medicamentos para morrer e 50% dos participantes afirma que em caso de doença incurável e em sofrimento recorreria a este procedimento. No entanto, quando consideradas as questões de fim de vida no seu conjunto observam-se algumas simplificações das controvérsias a que estão sujeitas as questões de fim de vida. Conclusão. Tratando-se de um tema atual e pertinente, tendo em consideração as características da amostra e os procedimentos de investigação utilizados, recomenda-se prosseguir estudos neste domínio dada a relevância das questões de fim de vida para a Gerontologia Social.
Context and objective. Aging is a multidimensional and progressive process of changes at the biological, psychological and social level, composed of gains and losses that occur throughout life (WHO, 1999). Longevity refers to the life span of an organism and depends on the progression of aging, as it imposes a limit on it sooner or later (i.e. the number of years to live). According to Zaterka (2015) there are differences between longevity and health, quantity and quality of life, that is, between the advancement of death (by means of regimes and drugs) and an effective extension of life. We need to reflect to what extent we should prolong this life and what its consequences are, being necessary to be aware of the limits of indefinite life extension, when there is no hope in the recovery of the dying, in which the best is an easy and just death. Death refers to the definitive cessation of life, and dying is an integral part of life, as natural and predictable as being born (Horta, 1999). The perception of death is different for each of us, either because of its diverse representations, conceptions and beliefs, being based not only on its occurrence, but also on the social and family context in which the individual is inserted (Gomes, 2013). Euthanasia, being an assisted death practice, means a good death, a death without suffering and without pain (Demarchi & Bastos, 2013). According to Suski (2012), euthanasia became an act of freedom in which the person with a terminal and incurable disease, verifying that he is close to death, considering that his life has no quality or dignity, there is the possibility of recognizing the person's right to decide to end his or her life. Medically assisted suicide consists of the help requested by the patient, so that another can help him to execute his death. Descriptions of euthanasia are cultural constructions that have evolved over the years, depending on the cultural and social environment (Jylhänkangas, Smets, Cohen, Utriainen & Deliens, 2014). In this context, the present study aims to identify the social representations of euthanasia and other aspects of the end of life in students of courses in the field of aging. Method. In this qualitative exploratory study, 75 students participated, being 53 undergraduate students in Gerontological Social Education and 12 master students in Social Gerontology. Data collection was carried out with a questionnaire composed of 12 open questions, aiming to explore aspects associated with death, euthanasia and medically assisted suicide. As for the data analysis strategy, content analysis was carried out, namely pre-analysis (material organization), material exploration (material coding, information classification and categorization) and data treatment (information interpretation). Results. In the present study, the participation rate was 76%, compared to the number of subscribers (n = 99), with the majority of participants being female (97%). Data analysis allowed the identification of three major themes, namely: euthanasia, death and medically assisted suicide. Regarding death, it was possible to identify death as a normative life event, being an event predominantly viewed in a negative way; 48% of participants think about their own death and 71% of participants think about the death of family and friends. As for euthanasia, for the participants it refers to a voluntary and / or consenting option to end life, about 59% of the participants are in favour of this procedure, 44% of the participants, as a family member favourable to euthanasia in a disease situation incurable and in suffering, accept the decision of the relative / patient, 45.33% of the participants as a patient in a situation of incurable disease and in suffering opted for euthanasia, 53.34% of the participants considered euthanasia to be a taboo subject and 65.34% of the participants is clearly in favour of legalization. As for the medically assisted suicide for the participants, this concerns the action of the person who takes drugs to die and 50% of the participants say that in case of incurable disease and suffering they would resort to this procedure. However, when considering end-of-life issues, some simplifications of the controversies to which end-of-life issues are subject are observed. Conclusion. As it is a current and pertinent topic, considering the characteristics of the sample and the research procedures used, it is recommended to continue studies in this domain given the relevance of end-of-life issues for Social Gerontology.
