Plano de resposta à catástrofe externa de um serviço de urgência básico

Abstract

A gestão de um evento de exceção/catástrofe que seja gerador de multivítimas assume-se como complexa, com necessidade de respostas que suplantam frequentemente os recursos materiais e humanos disponíveis nas unidades de saúde. Neste contexto, emerge a importância de uma intervenção planeada, que permita uma ação coordenada, integrada, eficaz e eficiente por parte de todos os profissionais e que se venha a traduzir em ganhos em saúde. O papel do Enfermeiro Especialista em Enfermagem Médico-cirúrgica no atendimento à pessoa em situação crítica passa pela dinamização da resposta em situações de emergência, exceção e catástrofe, desde a sua conceção até à ação, elevando a responsabilidade profissional para além da prestação de cuidados, assumindo um papel preponderante na elaboração de planos de resposta à catástrofe adequados à realidade da instituição onde se insere. Para a realização deste trabalho, adotou-se a metodologia de projeto, cujo principal objetivo se focou na elaboração do plano de resposta à catástrofe externa de um Serviço de Urgência Básico. Assim, assentou no diagnóstico de necessidades, através de um questionário dirigido à equipa médica e de enfermagem e de entrevistas informais a informantes estratégicos pertencentes à equipa, que permitiram elaborar um diagnóstico da situação existente. Os resultados encontrados revelam ausência de um plano de resposta à catástrofe e mostram, na opinião da classe médica e de enfermagem, a necessidade e importância da implementação de orientações estruturadas que permitem uma prática sistematizada e normalizada, em que cada profissional compreende o seu papel e as funções que lhe competem. Para além de valorizar a existência de um Plano de Resposta à Catástrofe Externa (PRCE), observou-se ainda preocupação dos profissionais com a falta de meios humanos/materiais disponíveis. Na continuidade da metodologia, foram desenvolvidas atividades que permitiram a elaboração e a validação do PRCE e colmatada a falta de recursos materiais, sendo adquiridos os meios para responder às exigências deste e dos eventos de exceção Relativamente ao domínio técnico e científico, constataram-se dificuldades na aplicação da triagem de catástrofe e na compreensão do enquadramento do SUB na gestão do teatro de operações, bem como na articulação com outras entidades, pelo que, foram implementadas atividades que permitiram suprimir as necessidades reconhecidas e dotar a equipa com as competências consideradas fundamentais. Da concretização deste trabalho em que se disponibiliza um instrumento de trabalho norteador da ação, espera-se contribuir para uma oferta assistencial de qualidade, que traduza ganhos em saúde através da diminuição da morbilidade e mortalidade, em contexto de catástrofe.

The management of an exception/catastrophe event that generates multiple victims is assumed to be complex, with the need for answers that often supplant the material and human resources available in health units. In this context, the importance of a planned intervention emerges, which allows coordinated, integrated, effective and efficient action on the part of all professionals and which will translate into health gains. The role of the Specialist Nurse in Medical-Surgical Nursing in the care of people in critical situations involves the dynamization of the response in emergency situations, exception and catastrophe, from its conception to the action, raising professional responsibility beyond the provision of care, assuming a preponderant role in the elaboration of catastrophe response plans adapted to the reality of the institution where it operates. In order to carry out this work, the project methodology was adopted, whose main objective was focused on the elaboration of the response plan to the external catastrophe of a Basic Emergency Service. Thus, it was based on the diagnosis of needs, through a questionnaire addressed to the medical and nursing team and informal interviews with strategic informants belonging to the team, which allowed the elaboration of a diagnosis of the existing situation. The results found reveal the absence of a catastrophe response plan and show, in the opinion of the medical and nursing class, the need and importance of implementing structured guidelines that allow a systematized and standardized practice, in which each professional understands their role and the functions that are assigned to it. In addition to valuing the existence of an External Disaster Response Plan (EDRP), professionals were also concerned about the lack of available human/material resources. In continuity of the methodology, activities were carried out that allowed the elaboration and validation of the EDRP and the lack of material resources was solved, with the means to respond to the requirements of the EDRP and exceptional events being acquired. Regarding the technical and scientific domain, difficulties were found in the application of disaster screening and in the understanding of the role of the Basic Emergency Service (BES) in the management of the theater of operations, as well as in the articulation with other entities, therefore, activities were implemented that allowed to suppress the needs recognized and provide the team with the skills considered fundamental. From the completion of this work, in which a work tool is available to guide the action, it is expected to contribute to a quality care offer that translates health gains through the reduction of morbidity and mortality in the context of catastrophe.
