Conhecimento dos enfermeiros do serviço de medicina intensiva, na área de prevenção de úlceras por pressão

Abstract

O relatório Crítico de Atividades insere-se no âmbito do Estágio de Natureza Profissional realizado no Serviço de Medicina Intensiva de um Hospital da região Norte, com vista à aquisição/aperfeiçoamento das competências específicas do Enfermeiro Especialista em Enfermagem em Pessoa em Situação Crítica e atribuição académica do Mestrado. Em Cuidados Intensivos a dependência das funções mais básicas coloca sobre o enfermeiro que presta cuidados a responsabilidade de manter as funções básicas de vida, prevenindo complicações e limitando incapacidades, tendo em vista a sua recuperação total. A escolha do local de estágio, assume um papel preponderante, uma vez que é através do contacto permanente com o doente crítico que a aprendizagem assume um papel ativo, e permite o cruzamento de experiências práticas e teóricas com o pensamento critico-reflexivo, tomada de decisão e a busca pelo saber. Este relatório encontra-se dividido em dois capítulos, sendo que o primeiro capítulo diz respeito às atividades desenvolvidas e competências adquiridas nos domínios de competências do Enfermeiro Especialista: domínio da prestação de cuidados; domínio da formação; domínio da gestão e domínio da investigação. A problemática que deu origem ao segundo capítulo deste relatório, percurso investigativo, foi a área de prevenção de Úlceras por Pressão. Observamos que o conhecimento dos enfermeiros não era unânime na prestação de cuidados, e que as estratégias de prevenção variavam de turno para turno. Deste modo, elaborou-se uma questão de investigação: Qual o conhecimento dos enfermeiros de um Serviço de Medicina Intensiva sobre prevenção de UPP?”. Foi utilizada a escala Pressure Ulcers Knowledge Assessment Tool para dar resposta à pergunta de investigação. Numa amostra de 61 enfermeiros, observa-se lacunas de conhecimento nas áreas de medidas preventivas, quer com a duração, quer com a quantidade de forças externas responsáveis pelo desenvolvimento de UPP, obtendo-se scores de 48,52% e 44,67% respetivamente. Os enfermeiros demonstram dificuldades no item da etiologia e desenvolvimento (51,37%), bem como classificação (47,27%). Apresentam conhecimentos desatualizados, bem como práticas atualmente não recomendadas. Os itens melhores pontuados foram o da avaliação do risco (74,59%) e nutrição (62,30%). Os enfermeiros obtiveram na Pressure Ulcers Knowledge Assessment Tool uma pontuação média de 50,54% das 6 áreas de conhecimento, o que revela um nível de conhecimentos insuficientes.

The Critical Activities report is part of the Internship of a Professional Nature carried out in the Intensive Care Unit of a Hospital in the North region, with a view to acquiring/improving the specific skills of the Nurse Specialist in Nursing in Person in Critical Situation and academic attribution of the Master. In Intensive Care, dependence on the most basic functions places the responsibility of maintaining basic life functions on nurses who provide care, preventing complications and limiting disabilities, with a view to their full recovery. The choice of the internship location assumes a preponderant role, since it is through permanent contact with the critical patient that learning takes on an active role, and allows the crossing of practical and theoretical experiences with critical-reflective thinking, decision-making decision and the search for knowledge. This report is divided into two chapters, with the first chapter concerning the activities carried out and skills acquired in the areas of expertise of the Specialist Nurse: domain of care delivery; field of training; domain of management and domain of research. The issue that gave rise to the second chapter of this report, an investigative path, was the area of Pressure Ulcers prevention. We observed that nurses' knowledge was not unanimous in providing care, and that prevention strategies varied from shift to shift. In this way, an investigation question was elaborated: What is the knowledge of the nurses of an Intensive Care Service about Pressure Injury Prevention?”. The Pressure Ulcers Knowledge Assessment Tool scale was used to answer the research question. In a sample of 61 nurses, gaps in knowledge were observed in the areas of preventive measures, both with duration and with the number of external forces responsible for the development of PU, obtaining scores of 48.52% and 44.67% respectively. Nurses demonstrate difficulties in the item of etiology and development (51.37%), as well as classification (47.27%). They present outdated knowledge, as well as currently not recommended practices. The best scored items were risk assessment (74.59%) and nutrition (62.30%). The nurses obtained an average score of 50.54% in the 6 areas of knowledge in the Pressure Ulcers Knowledge Assessment Tool, which reveals an insufficient level of knowledge.
