Estágio de Natureza Profissional: um percurso para o desenvolvimento de competências: registos de enfermagem no bloco operatório

Abstract

O presente relatório surge no âmbito do Curso de Mestrado em Enfermagem Médico-Cirúrgica e evidencia os principais contributos do Estágio de Natureza Profissional para o desenvolvimento de competências especializadas em enfermagem médico-cirúrgica. Para a sua elaboração, adotou-se uma metodologia descritiva e critico-reflexiva, suportada em pesquisa bibliográfica, com recurso a bases de dados e publicações periódicas. O Estágio realizou-se no bloco operatório central de um hospital da região norte de Portugal. O relatório encontra-se dividido em duas partes. Na primeira parte descreve-se a estrutura física do Bloco Operatório e respetivos recursos humanos e aborda-se também as atividades desenvolvidas durante o estágio bem como a respetiva reflexão crítica, no sentido do desenvolvimento pessoal e profissional necessário à aquisição de competências inerentes ao grau de mestre e especialista em enfermagem médico cirúrgica. Na segunda parte, descreve-se a implementação de um Projeto de Intervenção em Serviço (PIS) – Registos de Enfermagem no BO, utilizando metodologia de projeto, e subordinado ao tema: Registos de Enfermagem Perioperatória num Sistema de Informação em Enfermagem. Para a implementação do projeto realizou-se previamente uma análise SWOT, no sentido de se identificar fragilidades e forças subjacentes ao tema. Em resultado da análise verificou-se a deficiência de hardware e software adequados aos registos de enfermagem e também a eventual falta de formação dos profissionais sobre a Classificação Internacional para a Prática de Enfermagem (CIPE) e na utilização do Sistema de Informação de Enfermagem (SIE) a implementar. Em consequência, sugeriu-se a aquisição de hardware e software para o BO de acordo com os requisitos exigidos pela Ordem dos Enfermeiros e realizou-se o diagnóstico das necessidades formativas dos enfermeiros do BO através de um questionário aplicado a 42 elementos. Os resultados obtidos permitiram identificar défice de formação em relação à CIPE® e défice de formação relativo ao SIE. Posteriormente, planearam-se e executaram-se estratégias tendo em conta o diagnóstico de situação alcançado, tais como a realização de ações de formação aos enfermeiros do BO sobre os registos de enfermagem segundo a CIP, a implementação dos registos de enfermagem com base num SIE, a elaboração de um guia de orientações de registos de enfermagem, a produção de um flyer digital com identificação das principais atividades a registar durante o percurso do doente no intraoperatório e a disponibilização de ferramentas de software, estruturadas para o acesso rápido e organizado, de forma a estimular e a facilitar os registos. Para avaliar o projeto realizou-se uma auditoria retrospetiva interna aos registos de enfermagem efetuados entre 29 de março e 30 de junho de 2021. O resultado obtido foi objetivamente positivo, conseguindo-se desta forma implementar os registos de enfermagem CIPE no Bloco Operatório.

The present report was created within the scope of the Master's Degree in Medical-Surgical Nursing and highlights the main contributions of the Internship in Professional Nursing to the development of specialized skills in medical-surgical nursing. A descriptive and critical reflective methodology was adopted for its preparation, supported by bibliographic research using databases and periodical publications. The internship was developed in the operating room of a hospital in the northern region of Portugal. The report is divided into two parts. The first part describes the physical structure of the Operating Room (OR) and its human resources and also addresses the activities developed during the internship as well as the respective critical reflection, towards the personal and professional development necessary for the acquisition of skills inherent to the degree of master and specialist in medical-surgical nursing. The second part describes the implementation of an in-Service Intervention Project - "Nursing records in the OR", based on a project methodology in the OR, in line with "Perioperative Nursing Records in a Nursing Information System". For this implementation, a SWOT analysis was initially performed to identify the weaknesses and strengths in the department. It was identified shortcomings at the level of hardware and software supply for adequate nursing records and also a possible deficit in training in ICNP and in the Nursing Information System. As a result, it was suggested the acquisition of hardware and software for the OR regarding the specific needs and was performed an in-depth analysis of the personal registering skills through a questionnaire delivered to 42 operating room nurses. The results obtained allowed the identification of the specific training deficits in relation to the ICNP and to the nursing information system. Subsequently, different strategies were followed to pursue the main objectives, such as the carrying out of training actions on nursing records, according to the ICNP and the nursing information system; the development of a guide for nurses; the production of a digital flyer identifying the main activities during the patient's intraoperative journey; and the provision of software tools designed for quick reference and in order to encourage thorough and easier recordings. To assess the project execution, an internal retrospective audit of the nursing records was carried out for the period between March 29 and June 30, 2021. The result obtained was objectively positive, with this project it was possible to implement the ICNP nursing records in the Operating Room.
