Abstract: O fenómeno do envelhecimento (demográfico e individual) é um assunto de crescente interesse e preocupação mais ou menos informado por políticos e cientistas, que acordaram para o “Grey Power” quando este começou a impor-se nos diversos contextos. Um pouco por todo o mundo, foi-se percebendo que havia de facto um desafio com as potenciais despesas na Saúde e no sistema de Segurança Social e, simultaneamente, uma oportunidade com um mercado emergente de necessidades e serviços. O envelhecimento individual estabelece-se como um fenómeno biopsicossocial complexo, multidirecional, multidimensional e multicausal, regulado por ganhos e perdas e que contém o cunho de dois aspetos fundamentais que exercem influência, o lugar e o tempo histórico-cultural em que a pessoa vive. Nas últimas décadas a investigação tem reunido um corpo crescente e sistemático de evidências que permitem compreender muitas das dimensões deste processo, salientando o papel ativo e dinâmico da pessoa no seu próprio processo de desenvolvimento e envelhecimento. As questões da sexualidade e intimidade são transversais em qualquer idade, logo deverão ser também analisadas no envelhecimento, tal como ocorre em outras etapas da vida. A par de crenças e mitos acerca das pessoas idosas e da velhice, surge a vivência e a expressão da sexualidade, muitas vezes relegada para segundo plano, por se considerar que nesta etapa da vida a pessoa se torna num ser assexuado. Porque consideramos que a sexualidade e intimidade devem ser conceptualizadas e estudadas numa linha desenvolvimental, uma vez que estas vivências fazem parte integrante do desenvolvimento pessoal ao longo de todo o ciclo vital, o presente estudo de natureza qualitativa, de tipo fenomenológico, tem como objetivo compreender de que forma a sexualidade e intimidade é vivida e experienciada por idosos institucionalizados. Participaram no estudo nove pessoas de ambos os sexos residentes numa Estrutura Residencial Para Pessoas Idosas que foram entrevistadas com recurso a uma entrevista semiestruturada desenhada especificamente para o presente estudo. A análise conteúdo das entrevistas permitiu identificar a existência de um tema comum – Sexualidade e Intimidade na Velhice, que integra dois domínios: Vivência da sexualidade e intimidade e Posicionamento face à intimidade e sexualidade na velhice. O primeiro domínio integra sete categorias (1) Vivência da sexualidade; (2) Vivência da intimidade; (3) Motivos; (4) Constrangimentos à vivência da sexualidade e intimidade; (5) Estratégias pessoais e (6) Manifestações do envelhecimento e (7) Efeitos geracionais; e o segundo domínio agrega quatro categorias: (1) Conceção de sexualidade e intimidade, (2) Posicionamento face à sexualidade na velhice, (3) Conselhos e orientações e (4) Perspetivas sociais sobre a sexualidade na velhice. Face aos resultados obtidos é possível constatar a heterogeneidade na vivência da sexualidade e intimidade ao longo da vida, tornando-se evidente a presença de diferenças de género no modo como os participantes experienciaram e vivenciaram a intimidade e sexualidade antes e após a institucionalização. Especificamente, é evidente que a vivência da sexualidade e intimidade é influenciada pelas práticas educativas e valores morais/religiosos e pelos acontecimentos de vida, essencialmente o casamento como marco para o início da sexualidade feminina. Também ficou claro que a institucionalização por si mesma não parece constituir um fator de alterações na vivência da sexualidade e intimidade para os participantes no estudo, sendo a morte ou ausência do cônjuge ou parceiro e as fragilidades físicas apontadas como principais fatores para alterações nesta vivência. Os participantes distinguem o conceito de sexualidade do de intimidade, estando a sexualidade ligada diretamente à expressão de afetos (amor e carinho) e a intimidade suportada por sentimentos positivos como confiança e amizade. Em suma, a sexualidade e intimidade ao longo da vida e, principalmente, na velhice parece constituir-se como um processo heterogéneo, sendo consideradas estas vivências como positivas e necessárias mesmo na velhice e em ambiente institucional. Os participantes apresentaram um discurso reflexivo, com grande capacidade de abertura ao tema, desprovido de preconceitos, mas reconhecendo a sua existência por parte da Sociedade. Deste modo, os resultados permitem o desenvolvimento de orientações para a prática gerontológica, nomeadamente ao nível da formação dos profissionais nesta área, da educação da Sociedade e do trabalho direto com os mais velhos.
The aging phenomenon (demographic and individual) is a recent subject of concern more or less informed by politicians and scientists, that woke up to the” Grey Power”, when it began to impose on different contexts. A little all over the world, it was realized that there it was indeed a challenge with the potential expenditures on Health and the Social system and, simultaneously, an opportunity with an emerging market of needs and services. Individual aging is a complex biopsychosocial, multidirectional, multidimensional and multi-causal phenomenon, regulated by gains and losses, encompassing two key aspects which have influence, the place and historical-cultural time in which the person lives. In the last decades, the investigation has collected a growing and systematic body of evidence which allow understanding many of the dimensions of this process, emphasizing the active and dynamic role of the person in his/her own development and aging process. Sexuality and intimacy issues are transversal at any age, this way they should also be analyzed in the aging process, as it occurs in other life phases. Along with beliefs and myths about the elderly and old age, the experience and expression of sexuality and intimacy, often relegated to the background, because it is considered that at this stage of life the person becomes assexuated, emerge. As we consider that sexuality and intimacy should be conceptualized and studied in a developmental perspective, since these experiences are intrinsic to the personal development throughout the life cycle, the present phenomenological study aims to understand how sexuality and intimacy are lived and experienced by institutionalized elders. Nine people of both sexes residing in a Residential Structure for the Elderly were interviewed using a semi-structured interview specifically designed for the present study. The interviews content analysis allowed identifying a common theme - Sexuality and Intimacy in Old Age, which integrates two domains: Sexuality and intimacy experience and Positioning towards intimacy and sexuality in old age. The first domain integrates seven categories (1) Sexuality experience; (2) Intimacy experience; (3) Reasons; (4) Constraints to the sexuality and intimacy experience; (5) Personal strategies and (6) Aging Manifestations and (7) Generational effects; and the second domain encompasses four categories: (1) Conception of sexuality and intimacy, (2) Positioning towards sexuality in old age, (3) Advice and guidance, and (4) Social perspectives on sexuality in old age. Given the obtained results, it is possible to verify the heterogeneity in the sexuality and intimacy experience throughout life, and the gender differences in the way participants lived and experienced intimacy and sexuality before and after institutionalization becomes evident. Specifically, it is evident that the sexuality and intimacy experience is influenced by educational practices and moral/religious values and by life events, essentially marriage as the beginning of female sexuality. It also became clear that institutionalization by itself does not appear to be a factor changing the sexuality and intimacy experience for the participants, being the death or absence of the spouse or partner and the participants’ physical fragilities, identified as the main factors changing this experience. Participants distinguish the concept of sexuality from that of intimacy, with sexuality being directly linked to the expression of affection (love and affection) and intimacy supported by positive feelings such as trust and friendship. In short, sexuality and intimacy throughout life and, especially, in old age seems to be a heterogeneous process, and these experiences are considered positive and necessary even in the old age and in an institutional environment. Participants presented a reflective speech, with a great capacity for openness and reflection on the subject, devoided of prejudices, but acknowledging their existence on Society. This way, the results allow the development of guidelines for gerontological practice, namely on the training of professionals in this area, the education of the Society and the direct work with the elderly.
